Swaffham Community Hospital delivers quality, patient centered care within our 18 bedded unit, providing rehabilitation to a group of patients with varying complexities of needs. We accept adult patients of all ages who have rehabilitation needs. Our patients come to us following medical, surgical or orthopaedic treatment, sometimes directly from their homes.

Opportunities exist for further training and development within the role. You will be joining an enthusiastic, supportive, friendly multidisciplinary team.

The post involves rotation between day/night duties, full/ part time posts are available. Our focus is on continually improving the quality of care we offer to local people and on improving access to that care, helping people to move seamlessly from one service to another.

1. Implement care for patients on the as defined by the individualised care plan.
2. Provide a high standard of direct care for patients.
3. To work in a collaborative manner with all other staff working within the unit
4. To carry out direct patient care
5. To establish relationships with patients, family and friends that helps them to express their needs.
6. Communicate relevant information regarding the needs of patients, family and friends.
7. To document and sign all care interventions provided.
8. To be involved in the induction and mentoring of new staff where applicable.

Main Duties & Responsibilities

Management/ Organisational Responsibilities
- Ensure effective and efficient use of all ward/department resources.
- Provide leadership to all members of the ward, promoting effective team work.
- Be responsible for monitoring staffing levels and the skill mix to ensure that this is appropriate to meet the needs of patients, and taking initial action to rectify any identified deficits through ward manager/modern matron.
- Maintain a positive approach to achievement within the team.
- Ensure adequate provision and maintenance of equipment.
- Assist with the induction of new staff and students, acting as a mentor.
- Act as a role model giving guidance and support to other staff and students.
- Participate in problem solving related to work organisation.
- To ensure that patients and carers are aware of their current and ongoing health needs.
- To participate and promote clinical supervision.
