Amazing opportunities have arisen following a review of provision of mental health services for older people at West London NHS Trust. We are in the fortunate position of transforming and expanding our service to meet the needs of older people with mild to moderate mental health needs. We are looking for a Band 4 Link Workers to support older adult patients in the community with mild mental health needs, by offering practical advice and supportive solutions to enable them to live satisfying and independent lives.

The post holder will be working under the supervision of an Occupational Therapist. They will deliver a person-centred, enabling and occupation focused including social engagement. The aim is to support our service users to be more independent and less isolated.

The team includes Occupational Therapists, Psychologists, nurses and link workers.

The post holder will carry a defined caseload of patients with regular supervision from the Occupational Therapist/s.

They will support the assessment of patients and will help to carry out the treatment plan and interventions recommended by the multi-disciplinary team.

They will use their knowledge and skills to support patients and carers to access community services, to identify, requisition or follow-up some aids and equipment and will facilitate group activities.

This is an ideal post for someone with experience of working with Older People with physical or mental health needs to broaden their experience and develop their expertise to support the development of a new service.
