**What you’ll be responsible for**

**Improving quality and outcomes**
- To provide clinical assurance of the quality of the care provided by specialised services within the South London Provider Collaborative and Commissioning Hub (PCaDH)
- Supporting the South London Partnership (SLP) team in the assurance of specialised services compliance to applicable specification and guidance.
- The post-holder will provide the link with Clinical Commissioning Groups and their commissioned services within the hub to ensure there is a co-ordinated approach to care and discharge planning, which results in robust and effective care packages.
- Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved.
- To take action in respect of Safeguarding Policies and Procedures as required

**Enabling patient and public involvement**
- To act as a champion for patients and their interests and involve the public and patients in the policy development and decision-making of the SLP.

**Partnership and cross boundary working**
- To work across multiple providers within the NHS and Independent Sector
- To work with other Case Managers in the management of particularly complex cases and delayed discharges (including Transforming Care)
- To work in collaboration with services within the hub to manage relationships with CCG commissioned providers that provide the referral and aftercare components of the pathway.
- To work in collaboration with local authorities, third sector and other statutory bodies such as MAPPA, Police, HMPPS, MOJ and CJS in the management of effective usage of the specialised mental health services
- To work collaboratively with others and part of cross
- directorate teams to deliver successful outcomes for the SLP

**Leadership for transformational change**
- To model a collaborative and influencing style of working, negotiating with others to achieve the best outcomes. Embedding this approach across the SLP

**Using insight and evidence for improvement**
- To ensure that safe and effective care pathways are in place to facilitate the movement of service users through specialised services, including the identification and reporting of deficits within service provision
- To ensure the appropriateness of the placement for the children and young people
- Ensure case management systems and record keeping are maintained and kept up to date.
- The role will involve, but not be limited to you performing the above.
