Our Positive Pathways project engages with service users who have recently been, or are about to be, released from the prison system. The objectives are to assist service users to achieve and maintain suitable housing, stabilise their lifestyle and prevent future reoffending. Reporting to the Outreach Manager the Team Leader will lead a team of three Project Workers and will have key functions and duties as below.

Key Functions:

- Supervise the provision of the Positive Pathways Service.
- Ensure the Positive Pathways Service is provided to an agreed standard and within guidelines advised by SSSC and Care Inspectorate.
- Day to day responsibility for the effective running of Positive Pathways Service, including the achievement of agreed aims and objectives.
- Provide housing related support to those with complex needs and who have been liberated from Prison.

Main Duties:

- Ensure that Positive Pathways Service is delivered to the required standard, following the guidelines of the Local Authority Homelessness Team contract and Positive Steps internal policies and procedures.
- Provide support services to a caseload of Service Users within agreed support plans.
- Provide day to day supervision and guidance to team members.
- Induction, training, and appraisal of team members.
- Liaise with teams within Housing Support, Positive Steps Initiatives, and the Chance to Thrive project.
- Carry out risk assessments, safety planning and checks as required.
- Liaise with HMP staff, Criminal Justice staff and Local Authority Social Work
- Liaise with DCC Housing Options team
- Engaging with clients who have a history of disengaging from services, using a variety of techniques and approaches to build a relationship and maintain their tenancy.
- Conduct case review meetings with Project Workers when required.
- Produce reports on service outcomes and provision for funders and in house quarterly reviews.
- Maintain accurate manual and computer-based client records (in accordance with data protection and Positive Steps procedures).
- Ensure confidentiality of Service Users, Positive Steps and its staff is maintained.
- Develop own knowledge/practice and attend training as agreed with senior staff.
- Carry out any other duties applicable to position and agreed by senior staff.

Special Terms:
Must have use of a legal roadworthy vehicle with appropriate insurance.

**Salary**: £24,000.00-£26,000.00 per year

Schedule:

- Monday to Friday

Comprehensive risk assessments and systems covering both office work and work undertaken away from the office.
