We are looking for someone to join our team of Community Navigators in the county of Denbighshire. To provide a link between Health and Social Care professionals, Denbighshire citizens, their family, and carers, and also sources of support within the community and third sector. This approach can also be known as ‘Social Prescribing’. Social Prescribing can be described as ‘a mechanism for linking citizens with non-medical sources of support within the community.

**A Day in the life of a Community Navigator**
Every day you will work closely with the Community Resource Team, the Talking Points, the Talking Points and SPoA Wellbeing Coordinator and people from a range of local services.
You will engage with Service Users at Talking Points or in their normal place of residence or another setting to carry out needs assessments and develop support plans utilising the “What Matters” approach.
You will be working in the community and have a knowledge of services (both statutory and third sector) which are available to support the citizens of the county of Denbighshire. You will be a source of current, accurate and timely information and knowledge about a comprehensive range of support available within the community. Community Navigators support people to make connections with a range of local support. They work closely with the Community Resource Teams, ‘Talking Points’, Complex Disability Team, Primary care, Community Mental Health Teams and local people from a range of local services.
