**_ _**Role Snapshot**Working for South Norfolk and Broadland Councils, you will be delivering a social prescribing service to patients of Fakenham Medical Practice. You will be referred patients who are presenting with social, rather than clinical, needs or whose clinical condition is being adversely affected by social factors. You will work with patients to find solutions to their problems and thus contribute towards ensuring equality of health outcomes.

Norfolk and Waveney has a well-developed Integrated Care System in which the NHS, Local Authorities and partners from the VCSE sector, work together to maximise health outcomes. The Community Connector Team which you would be joining, is well-established with a supportive team of 28 colleagues. Training, support, and practice supervision are always available.

**What you’ll be doing **
- Taking referrals from GPs and other Primary Care professionals
- Assessing referrals in the Practice and co-producing plans of support with them
- Developing relationships with a wide range of agencies with whom you will develop plans of support for patients
- Maintaining clear and accurate records of your interactions.

**You must have **
- A knowledge of benefits/welfare rights systems
- Experience of working with people in distress
- A working knowledge of common mental health problems
- Excellent communication skills, verbal and written
- The ability to form and maintain professional relationships
- Personal resilience and a sense of humour.
