The One Care Home Team’s purpose is to provide high-quality and safe re-active and pro-active face to face and virtual MDT support to care homes and their residents.

The post holder will work closely with colleagues from the acutes and care home managers within the borough of Barnet. You will have the chance to rotate in the two hospitals the TA is covering.

We offer regular clinical supervision, guidance and training for personal and professional development in a friendly and supportive environment. This includes a program of in-service training and other training and development opportunities.

Experience of negotiating and liaising within a multi-disciplinary team. Experience of working with people with complex needs. Experience of undertaking comprehensive health needs assessment and developing complex care plans

Work within acute hospitals to identify patients with complex care needs to support and facilitate timely and appropriate safe transfer of care following discharge from hospital to avoid unnecessary delays.
Support the increase in bed flow and status

Provide discharge planning for diverse and highly complex patients

Guide and direct multiple agencies in order to achieve good patient care

Demonstrate a specialist understanding of discharge planning and resource to staff and other members of multi-disciplinary and multi-agency teams
