**The outcome of the council’s Total Pay and Reward review means the full-time salary for this position from 1**st** April 2024 will be £27,799 to £28,282. The salary will be pro rata for part time and term time vacancies.**

An exciting opportunity has arisen for a part time Care Officer to join Haltemprice Community Mental Health Team, which provides support to people experiencing complex mental health difficulties, who live in the community and who require secondary level support and intervention. The role will involve joint working with other services within the Council (such as Adult Community Wellbeing and Safeguarding teams) and the NHS Humber Teaching NHS Foundation Trust. It will involve working with other partner organisations, both statutory/voluntary and working with families/significant others to provide a high quality, professional service to people experiencing significant mental health issues. The role will include assessing, and reviewing a person’s needs in terms of support, advice, and care. The post holder will be part of a multi-disciplinary team working alongside a range of other professionals.

Other duties will involve acting as Duty officer, providing crisis support/intervention, risk assessment/management and formulating and reviewing packages of support and care in line with Local Authority procedures and the Care Programme Approach. You will be required to store and retrieve electronic data on the adult recording system (Azeus Care) and the NHS Lorenzo system. You will also be expected to contribute to and develop the service in line with national and local drivers.

The team provides support to people living in the East Riding area and therefore access to transport to carry out your role is crucial.
