NHS England guidance published in December 2021 advises the use of a one off discharge grant, enabling patients to be discharges from hospital in a timely manner. This has offered an opportunity for the Integrated Care Boards, Trust and Local Authorities to collaborate to develop systems that would support the delivery and coordination of the one off discharge grant.

The initiative helps to support the timely discharge of patients ensuring that patients are discharged in a timely manner, improving patient experience and reducing hospital acquired infections.

We are looking for an enthusiastic individual to take up the role as the Discharge Grant Project Coordinator. The post holder will be based at St Helier hospital and will coordinate the one off discharge grant process, work closely with the acute hospital and the Home First+ team.
