**We are looking for an experienced and passionate Wellbeing Practitioner to work as part of our Sequoia Tree Service.**

Our Sequoia Tree service (Sequoia) is a brand new offer across Bristol, North Somerset and South Gloucestershire (BNSSG) and is delivered in partnership with Avon and Wiltshire Partnership (AWP) Trust and will require the post holder to work cohesively with the AWP team as one integrated service including attending multi disciplinary team meetings, reflective practice sessions and meeting in shared co-locations regularly.

The service supports adults (aged 18+, with an aspiration to include from age 16+ as the service evolves), who have complex emotional needs, personality difficulties and people with a personality disorder diagnosis’. The service sits at a primary care level within the NHS i.e. GP level. By working in an integrated way with AWP colleagues, the service offers both clinical and holistic support. The support is delivered in conjunction with peers and people with their own lived experience, developing an offer within the community and working with them to build meaningful connections.

**How you will make a difference**

As a Wellbeing Practitioner I am part of a team providing an excellent quality of support to people using the Sequoia Tree service.

I work within a multi disciplinary, innovative and creative team which provides high-quality, safe and person-centred support in the community - most importantly delivering great outcomes for people who have complex emotional needs, personality difficulties and people with a ‘personality disorder diagnosis’.
