Lilibeth Cousins Ltd are looking to recruit one Care Coordinator for The City of Bedford Borough Council. The post is responsible to the Cluster Home Care Manager for the delivery of integrated health and social care home-based care services in a geographical area within a locality cluster.

**Key Responsibilities**

Contributing to the effective delivery of locality home care services will include:

- This includes ensuring that the service delivers a high standard.
- The service will be responsive, provide safe and effective care, work with individuals, their carers.
- To establish, develop and maintain excellent relationships with independent sector care at home providers.
- To contribute to a team which promotes the values and principles of National Care Standards.
- To contribute to change and innovation and the development of a positive culture within the service. Experience/ Skills Skills:

- HNC in Social Care and SVQ3 in Health and Social Care or a professional qualification in an appropriate field is essential (e.g. nursing qualification, OT qualification, Diploma in Social Work). Experience:

- Excellent people-management and organisational skills (e.g. negotiation, reasoning, conflict management, and excellent written and verbal communication skills including working with people with communication difficulties).
- An understanding of the relevant legislative and policy context (e.g. including National Care Standards, adult protection processes and frameworks, integration, personalization and self-directed support).
- Registration with SSSC is required, a suitable management qualification will be required within the registration period.
- Ability to demonstrable knowledge of working with diversity and equal opportunities.
- Develop and coordinate healthcare programs
- Communicate between staff, patients and family
- Handle patient case management and education
- Recruit and train staff and create schedules
- Develop patient goals and monitor progress
- Maintain patient confidentiality and quality care

**Requirements and skills**
- Proven work experience as a Care Coordinator or similar role
- Highly organized with a strong ability to manage different priorities
- Ability to work with others and use resources to problem solve
- Strong customer service skills
- Effective verbal and written communication skills
- Medical assistant certification preferred
