Care Coordinator

Danygraig Domiciliary care agency is looking to recruit an experienced care coordinator to support a busy office in the delivery of community based social care. The person we are looking to recruit must have at least two years of care experience preferably in a senior role.
The role includes:
- To coordinate weekly customer rosters
- To plan all client calls and appointments
- Monitoring the Electronic Call Monitoring System.
- To carry out client reviews alongside the managers
- To work in a professional manner with family members and other people involved in the persons care to ensure holistic support is provided.
- Communicate and liaise with all relevant health professionals;.i.e. Social Workers, brokerage and others.
- Support the business to grow by working with brokers to take on new packages of care
- To provide oncall cover on a rota basis
**Essential Requirements**
- At least one years experience in working within domiciliary care setting
- 1 years experience working at a senior level within a care setting preferably community based care
- Experience of being a scheduler and understanding software systems used in rostering client calls is essential.
- Excellent planning and organizational skills,
- QCF 2 in social care
- To have a working knowledge of the needs of vulnerable people
- Excellent IT skills
- Car owner driver
- Excellent communication skills
- Excellent telephone manne
- hours of work between 6:30am-2:30Pm
**Desirable requirements**
- Experience in a care coordinators position
- QCF in social care 3 or 5
- SCA Wales registered
