**About the Role**:
GP Healthcare Alliance is a federation of 18 GP practices in Castle Point and Rochford. In partnership with the local primary care networks (PCNs),we are hosting an innovative service to recruit a population health practitioner to support our PCN and member practices.

The main aim of this role is to increase the health and wellbeing of patients, through improved screening, identification, and disease management. This will entail a mixture of clinics and home visits, as directed.

The post holder will work collaboratively with the general practice and wider primary care network team, to meet the needs of patients, support the delivery of PCN objectives, act as a role model to junior staff and to demonstrate a caring and compassionate attitude.

This role will involve working autonomously, but with the support of a multi-professional primary care team made up of doctors, nurses, and clinical pharmacists.

This is a full time post, working 37.5 hours per week, covering the Rochford Primary Care Network which is made up of the following practices:

- Ashingdon Medical Centre
- The Practice Leecon Way
- The Puzey Family Practice
- Wakering Medical Centre

**You will**:

- Be registered with the Nursing & Midwifery Council

**Or**
- Have a postgraduate degree in physician associate studies, from an accredited PA programme.
- Have a bachelor’s degree in life science/biomedical/nursing or allied health science.
- Be a member of Faculty of Physician Associate Managed Voluntary Register (PAMVR).
- Have a valid National certificate and recertification every six years as stipulated by PAMVR.

**Or**
- Be a registered paramedic with HCPC registration and experience in an emergency care setting.
- Have a qualification in advanced practice or currently working towards.
- Have a minimum of three years post registration with demonstrable experience of using diagnostic skills.
- Have knowledge and understanding of the Primary Care Network Direct Enhanced Service, local alliance vision/values and how primary care fits in with the NHS Long Term Plan

GP Healthcare Alliance is a company, originally formed as a GP Federation, made up of 18 practices who work together to provide healthcare for our practices and patients, in Castle Point, Rochford and Rayleigh, Essex. All practice members remain independent contractors, but work collaboratively to improve healthcare services within our area.

We provide a range of primary care and community services to our local residents, various ‘at scale services’ for practices, as well as professional staff training. We meet nationwide challenges with local knowledge and expertise, tailoring our services to reflect the needs of the communities in which we work.

We will develop new services, where we believe we can improve the patient experience and deliver efficiency savings to the NHS. We work with local NHS clinicians in the delivery of the services, thereby supporting the retention of strength and depth within the local NHS clinical teams.
