Are you passionate about supporting people in your local community and working to achieve goals with our service users to live independently in their own home?

We are looking to recruit 1 individual on a permanent basis 30 hour contract who truly care and are committed to making a difference in an individuals life.

Being an Assistant Reablement Coordinator means supporting people in their own homes for a short space of time to assist them to reach their full potential, this may include managing a variety of aspects of daily living skills, set clear and achievable goals to help people rebuild their former independence and confidence to sustain independent living at home and in their communities.
