To provide a key role in supporting the Head of Commissioning and Strategy to ensure that the ICB is meeting is statutory responsibility in meeting the needs of Children & Young People with Autism and or a Learning Disability who are at risk of an inpatient admission. To take the lead role in developing pre DSR pathways with key health, education and local authority partners. To bring learning from the development of pre DSR processes to the Keyworker project, linking in both with the operational team and Strategic Leads To provide the interface with the Key Worker service to support children and young people who are in tier 4 services or are at risk of being. To act as a co-ordinator between clinical decision making, localities and families and be able to challenge appropriately.
To co-ordinate support to a child and their family and hold partner agencies to account in terms of their responsibilities To have the ability to think flexibly and creatively in determining the most appropriate support to meet the child's needs. To be up to date on and provide training on legislative changes relevant to the remit of this role. To support the development and delivery of organisational objectives and priorities. To provide a key role in advising and assuring the ICB/ICP Executive at Place level and on the delivery and performance of the organisations commissioning plans and national performance measures.
To contribute to the strategic development of the system to ensure the delivery of The Long-Term Plan and Transforming Care plan at Place with links into the wider ICB systems.
