**Role Purpose**:
This role will be working on our Borders Brain Injury Service (BBIS), providing rehabilitation-based interventions to support individuals the progression of our service users to rebuild their lives following a brain injury. The service is delivered through a person-centred approach, to achieve progression for each service user appropriate to their individual needs following an in-depth initial assessment and action planning. The position is based in Galashiels and working throughout the Scottish Borders, providing an outreach service in the community. This role will include the promotion of the BBIS to potential referral organisations and individuals in order to maintain caseload levels and attract starts on to the programme. It is essential that the Rehabilitation Worker has experience of working with people who have a brain injury, neurological conditions, mental heath or complex needs, or a similar cohort of individuals.
**Key Responsibilities**:
As the Rehabilitation Worker, you will be responsible for the recruitment, induction, and initial assessment of service users and to deliver a person-centred service to individuals on your caseload. You will be required to:
- Identify the specific needs for each service user using a ‘Well-being Star’ tool which will then feed into their individual action plan and be agreed with each individual.
- Conduct both one-to-one sessions, exploring the consequences of the individual brain injury, the effect on their functional abilities, and develop and implement specific strategies to assist with the process of rehabilitation.
- Plan, develop, implement, and review appropriate interventions to promote community integration for individuals with their families and carers.
- Provide brain injury awareness information sessions for service users and their families/carers.
- Conduct regular progress reviews with each service user.
- Develop and sustain positive and effective relationships with current/new public and third sector partners, including NHS, Borders and Lothian, Third Sector and Public Sector organisations.
- Liaise and develop professional relationships with a range of external agencies at forums to promote the service.
- Complete and process relevant paperwork in line with contractual requirements
This is not an exhaustive list; additional duties may be required as deemed necessary by DCF.
**Education**:
- Educated to HNC (SCQF Level 7) or equivalent is specified as desirable but relevant experience is of greater weight.
**Experience, Skills, and Knowledge**:
- Recent and relevant experience of working with people who have experienced ABI, TBI or Neurological condition.
- Experience of assessing an individual’s needs and translating then into realistic and achievable goals.
- Experience of leading 1:1 sessions and group
- Experience of developing, delivering, and sustaining creative and stimulating activity-based group sessions.
- Ability to influence and negotiate required outcomes and proactively promote services to a range of individuals.
- Experience of coaching or supporting individuals to achieve their potential.
- Experience of partnership working and networking with other agencies.
- Excellent communication and interpersonal skills.
- Possess high standards and have excellent organisational skills, being able to prioritise workloads and deadlines.
- Ability to deliver presentations to groups internally and externally.
- Ability to work unsupervised, independently and on own initiative.
- Highly organised with the ability to manage own workload and prioritise accordingly.
- Good level of computer literacy and IT skills.

- Flexible approach to working hours, duties, and arrangements and able to work within agreed timescales.
**Quality and Continuous Improvement**
- To maintain accurate service user records to ensure evidence of their rehabilitation journey.
- Report on caseloads and areas of responsibility as required, including starts, initial assessment, reviews, service user achievements and producing good news stories.
- Always maintain the highest standards of participant care.
