As the Head of Patient Safety and Risk, you will play a crucial role in ensuring the safety and well-being of our patients while nurturing a culture of continuous improvement. As the Head of Patient Safety and Risk, you’ll lead initiatives to enhance patient safety and drive positive changes across our organisation, shaping a safer and healthier future for all.

**Key Responsibilities**:

- Lead the implementation of the Patient Safety Incident Response Framework, ensuring effective governance and assurance processes are in place
- Ensure compliance with Being Open and Duty of Candour requirements, involving patients and their families in investigations and sharing lessons learned
- Collaborate with various stakeholders to respond to external reviews and inspections, including the CQC and internal audit
- Facilitate organisational learning, ensuring that lessons from incidents are translated into actionable improvements
- Provide guidance and support to colleagues conducting investigations and action plan development
- Maintain and update the Patient Safety Incident Response Plan and Policy
- Lead specific patient safety improvement projects and undertake deep dives and thematic reviews
- Promote and develop a safety culture, encouraging staff to actively engage in safety initiatives
- Coordinate and participate in hospital-wide safety events and service quality assessments
- Produce regular patient safety and risk reports for hospital committees
- Foster collaborative relationships with external organisations and regulators for incident reporting and investigations

**Person Specification**:

- Degree or equivalent qualification and/or significant relevant experience
- Qualification in a risk-related discipline at diploma or equivalent (desirable)
- Healthcare-related professional qualification (e.g., nursing, medical, or AHP) (desirable)
- Significant experience of working at a senior level within the health service (or equivalent)
- Experience in patient safety, quality, risk management, or clinical fields
- Experience of leading and managing change and working within multi-disciplinary teams
- Knowledge and understanding of clinical and non-clinical risk assessment and management
- Knowledge of the Serious Incident Framework and the Never Event Framework (desirable)
- Excellent organisational, communication, and problem-solving skills
- Ability to build effective collaborative relationships and influence others positively
- Confident, self-motivated, and enthusiastic with the ability to motivate others
- Flexible and able to work outside regular hours to accommodate clinical staff meetings
