**PRINCIPAL DUTIES**:
The Successful applicant will be responsible for the planning and co-ordinating meaningful destinations for our learners post college. such as employment, further education, day services and accommodation for learners at our East Riding (Beverley) and Lincoln Campuses.

KEY TASKS:
1. To be responsible for co-ordinating suitable transition out arrangements for College learners, liaising with college staff, learners, families/carers and external professionals as appropriate.

2. Provide high quality transition out reports for annual reviews and attend the reviews.

3. To be responsible for planning, arranging and undertaking timely IAG tutorial sessions with learners.

4. Produce reports and any other documentation as required for the Local Authority; including destination statements.

5. Taking a key role in the organisation and execution of Open Days/Evenings.

6. Liaising with parents, careers advisors and other professionals to ensure the timely completion and collation of high quality destination/transition out information.

7. To track, monitor and evaluate leavers destinations; this may include flexible working, traveling and overnight stays.

8. Working in conjunction with the Programme Area Managers to ensure transitions out plans are compatible with the learners planned learning outcomes.

9. Working in conjunction with other key college staff to ensure that any required support or specialist input is identified and then completing relevant referrals for appropriate services.

10. Ensure that all opportunities to positively raise the profile of the College are fully exploited to the benefit of the College and the Trust as whole.

11. Produce any correspondence related to undertaking the role as required.

12. In conjunction with team colleagues, ensure that the College destinations database is updated and amended in a timely manner.

13. Attend and contribute to internal and external meetings as required; providing reports as necessary.

14. Ensure all internal records are maintained and filed.

15. To participate in marketing events as required.

16. Accompanying and supporting learners to attend meetings and visiting prospective placements as part of the transition out process.

17. To implement and promote the Linkage Policy for Equality and Diversity

18. To undertake appropriate personal and professional development.

19. To be personally responsible for your own health and Safety and to be aware if obligations to other staff and customers in accordance with legislation.

20. To undertake any other reasonable tasks as requested by the line manager.
