Rugby, United Kingdom Coventry and Warwickshire Partnership NHS Trust Full time

Senior Primary Care Mental Health Practitioner - Band 6) (ARRS)

Rugby

This is an exciting opportunity to build on the existing MH practitioner offer across the primary care networks & to support GPs in the ‘RUGBY’Primary Care Network (PCN). This compromises of 12 GP surgeries with a population of approx. 112,000. You will be employed by Coventry & Warwickshire & Partnership Trust but be embedded within a group of GP practices with a focus on supporting local patients living in Rugby.

We’re pleased to be able to consider flexible working, with arrangements including agile working, compressed hours, or staggered hours.

This is an ideal post for those professionals who are proactive, dynamic, & passionate about innovation & providing a high-quality holistic approach to the care of people who have been identified as in need of mental health support in a primary care setting.

This is an exciting opportunity that recognises that placing specialist mental health staff in GP practices & promoting early assessment & treatment in a recovery focused way will help people with mental health problems achieve the best possible outcomes.

The role will include providing navigation to help GP’s make appropriate referrals to the relevant community and secondary care services - bridging the gap between primary care services such as IAPT and secondary care and working in partnership across the system, including voluntary sector colleagues.

It will include reviewing patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialised advice concerning care when appropriate and liaising with GPs and MH teams.

To offer brief interventions as needed to people who have mental health needs, but that do not need a referral into Secondary Care MH services. e.g., problem solving, decision making, building resilience, building on personal strengths, risk de-escalation, medication advice, single session assessment and prescribing if a registered NMP. This will include facilitating short term case management offering professional advice, joint support and information.

To work closely with social prescribers and other professionals within the PCNs multi-disciplinary teams, to support the patient to access the right services in the right place and at the right time.

Community mental health is undergoing significant transformation and these posts are part of the transformation to support strengthened partnerships between primary and secondary care.

Our CMH Transformation Programme means that we are seeking new and innovative ways to improve the services we deliver in a more holistic, person-centred way, which treats both physical and mental health. Come and join our expanding team at this time of growth and investment in our staff and services. You’ll have the opportunity to contribute to the transformation as you progress your career with us. We nurture our staff to be the best they can be and provide continued personal and professional development opportunities

Our dedicated People Hub brings together all our support services including COPE, our in-house counselling service, emotional support through Staff in Mind, and much more. Our Employee Assistance Programme has a 24/7 helpline for advice and guidance with anything that is on your mind, work-related or not, as well as a salary sacrifice scheme.
