An exciting part time opportunity has arisen for an advanced clinical practitioner to join the Acute Frailty Team at Imperial, on a part time, fixed term contract from September 2023. The Acute Frailty Team currently provide an in reach service into the Emergency Departments (ED) at both our St Mary’s and Charing Cross sites, assessing frail patients using a comprehensive geriatric assessment (CGA) framework, with the aim of admission avoidance.

The post holder must a qualified advanced practitioner at diploma level or above. The role will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the ED and SDEC settings. The successful applicant will have a passion for older people’s care. Experience of completing a CGA is desired but not essential. This is a fantastic opportunity to gain experience within this ever expanding area of practice, gaining skills that can be applied to other speciality areas where older people are being treated.

Please note that this position is for the St Mary’s site but the post holder will need to be flexible and able to work at Charing Cross as required.
- To deliver Comprehensive Geriatric Assessments (C.G.A) to patients who meet the referral criteria. This holistic assessment will include clinical, social, functional, emotional and cognitive elements.
- To use the CGA knowledge gained, initiating and advocating plans which not only enables an early discharge but also meets the needs of the patient and reduces the likelihood of an unnecessary readmission
- The role requires understanding of the particular requirements of elderly patients within an acute setting and the knowledge and skills to assess patients with commonly recognised admission criteria E.g. Falls, Urinary Tract and Lower Respiratory tract Infections, Delirium and Dementia.
- The role demands the commitment to provide clinical support to patients, their families/carers and health professionals involved in the patient journey. This support will when appropriate involve discussions on Universal Care Planning
- The post holder will advance the role by integrating the four key pillars of advanced clinical practice, service management, education and research whilst collaborating closely with other members of the multi-disciplinary team to develop and monitor standards of care.
