**Job overview**:
Launceston Community Hospital is an 18 bedded ward with a multi-disciplinary team providing rehabilitation for patients in Cornwall. We provide an innovative opportunity to support our patients to receive high quality care and remain close to home.

**Main duties of the job**:
We are looking for someone who is able to work under their own initiative, to motivate and encourage patients within a positive and active environment. The role will require good motivational and physical activity skills and the ability to use own initiative to provide activities. You will be working closely with the multidisciplinary team to provide meaningful activities for patients.

Individuals who are self-motivated and can work independently are encouraged to apply.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
