**Job Title**: Community Health and Wellbeing Worker (CHWW)/Health and Wellbeing Coach
**Responsibility**: We are looking for a Community Health and Wellbeing Worker to join our team and work to support our patients in the community. We want you to work in the heart of our community and help improve the health and wellbeing of the young, the old and their families.
Community Health and Wellbeing Workers are passionate role models and provide advice and signposting to everyone within their communities, enabling them to make positive choices around health and wellbeing. You’ll meet people face to face, providing support and advice to those that need it. You’ll help people find local health services, offering to help them make appointments or sign post them to local services to support their needs. You’ll also serve as a link between health, social services, and our community to ensure all households receive the best possible care and service. The role involves working very closely with the practices and the multidisciplinary team
(MDT) within the OX3+ PCN (Hedena Health & Manor Surgery). The role is pivotal in
**Hours of work**:20-24hrs per week
**Salary**:£22,383 (pro rata)
**Job Summary**:
Community Health and Wellbeing Workers are local people who care about the health of their community and those around them. As a Community Health and Wellbeing Worker (CHWW) you will be a frontline primary care and public health worker employed from the local community. The outreach work of a CHWW is essential in addressing the rising inequities in health and social care and in identifying unmet need in the community. The CHWW performs a variety of duties with the aim of improving the health and wellbeing of the communities they serve. They play a significant role in increasing health promotion, through a range of activities including outreach assessments, raising awareness, community education, signposting, informal counselling, and support. Their bridging role between health and social services and the community, will help remove barriers to people accessing the support and services that meet the needs of the local community. Community
Health Workers are positive role models and provide advice and signposting to people living in their own communities, to enable them to make positive choices around health and wellbeing. This is an exciting opportunity to shape a new job role new to the UK.
**Key Duties & Responsibilities**:
Monthly household visits (or more frequent if the household need requires it) within a defined geographical area (up to a maximum of 120 households) to assess the health and social needs of everyone within a household, adopting a proactive and holistic approach when supporting the local community.
Community Health Workers will:
▪ Make contact with each household in a defined area on a regular basis, face to face or virtually as appropriate, listening and discussing health needs on each visit.
▪ Identify vulnerable households or individuals in conjunction with health care teams.
▪ Identify health and/or social care needs in conjunction with health care teams.
▪ Act as an advocate to help households navigate the health and social care systems, access appropriate services and remove barriers to accessing services and resources.
▪ Manage their time to ensure that visits are completed.
▪ Adhere to the Lone Worker Policy when carrying out visits to households.
▪ To work collaboratively with colleagues across health, social care and the third sector to provide patient-centred, integrated care.
▪ Develop supportive relationships with local VCSE organisations, community groups and statutory services, to make timely, appropriate, and supported referrals for the person being introduced.
▪ To liaise with the two practices (Hedena Health & Manor Surgery) and, where practicable, to standardise processes across the PCN.
▪ To promote health coaching and social prescribing amongst the PCN and networked practices.
**Educational**
- To provide personalised support to individuals, their families, and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in manging their own health and well-being, to live independently and to improve their health outcomes through the following: o Providing interventions such as self-management, education, and peer support o To signpost household members to the correct healthcare professional/service. o Supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual. o Working with the social prescribing service to connect them to community-based activities which support their health and well-being.
