The Home First Project Support Officer is a full-time weekday post however could be flexible and consider minimum of 30 hours. This is a secondment post for 12 months and fixed term will also be considered.

As Home First Project Support Officer you will be responsible for supporting the delivery of key projects, with the aim of preventing avoidable hospital admissions. Ensuring treatment and care are provided close to home. Discharging patients when ready to the most suitable place, preferably their home.

To support Home First project leads in the development and delivery of priority workstreams and contribute to the effective running of the Home First programme office.

To take the lead as directed by the Home First Programme Manager for delivery of key project tasks, including developing and maintaining project plans and project control documents.

Excellent communication and organisational skills are a must, and some experience of supporting projects would be a benefit but not essential.

You will have the ability to build and develop collaborative relationships with different health and care partners to formulate and adjust delivery plans.

You will be comfortable with complexity and able to use data to analyse options and make recommendations which may involve the negotiation with or persuasion of others to meet project objectives.
