The Cancer Tracker /MDM Coordinator will work with a range of clinical and non-clinical staff in our cancer multidisciplinary team in the Women’s and Children’s service, but will be expected to provide cover for the Gynaecology Pathway Manager during periods of annual leave or sickness absence.

The post-holder will also be expected to carry out the following:
Work closely with all members of the multidisciplinary team and associated clerical staff. Ensuring the timely movement and tracking of cancer and potential cancer patients through their journey from referral.

Identify any barriers to individual patient pathway and/or potential breach of waiting time guarantee and escalate to the appropriate manager in accordance with local policy.
Facilitate the collection, analysis and presentation of audit data recording key data items to assist in monitoring the care of patients.
Coordinate regular multidisciplinary team meetings (MDM’s) for patients diagnosed with cancer (s), ensuring the journey for each cancer patient runs smoothly and is progressed in a seamless manner.
To track the progress of each cancer patient.

To record key data items to assist in monitoring the care of patients.

To ensure that follow-up action is implemented and that information is relayed to the patient and the GP within the time frame stipulated.

You will require strong written and oral communication skills and the confidence to negotiate with senior members of staff.
