The Support Planner role will join a small and friendly team based in Nottingham City. You will need to travel for this role but will either receive a travel pass or mileage paid and parking passes. Your office-based work will then be hybrid office and home based.

The Cluster Team has 26 locations around central Nottingham. We support individuals with multiple complex needs in their own tenancies which include self-contained flats and shared housing. Our team is made up of Support Workers and Support Planners who aim to support tenants toward attaining permanent accommodation. We provide support in areas including housing, money management, substance misuse and liaise with external agencies who specialise in these areas.

**Duties & Responsibilities**
- To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs.
- You will have caseloads with Tenant’s in properties across the city (mileage system for car drivers and public transport paid).
- Prepare Support plans.
- Sign post Tenant’s onto appropriate services for substance misuse management & health.
- Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
- To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
