The aim of the Home from Hospital service is to provide very short-term (2-3 weeks ) social and practical help to those vulnerable people who feel they may need additional assistance following discharge from hospital - in particular those who may have limited support in their local community.

This is a community based role job sharing with the other coordinator, working Monday to Wednesday in the day, visiting people in their homes in Wokingham Borough. Outside of visits the coordinator would be in the Reading office once a week or working from home. Routine hours are 8.30am to 4.30pm.

**Main Duties**:
1. To coordinate the Wokingham Home from Hospital scheme, ensuring that clients are supported for 2-3 weeks following discharge from hospital

2. To liaise with the Home from Hospital Service Coordinator job share to ensure that a seamless service of care is maintained.

3. To receive referrals from hospital contacts and ensure that discharges are supported within 24 hours of notification.

4. To carry out risk assessments and set up person centered support plans that enable individuals to live in their own homes following discharge from hospital

5. To enable individuals to maintain independence by assisting them to continue with everyday tasks/routine at home (shopping, prescription collection/arranging, paying bills, GP appointments and light household tasks).

6. To provide emotional support and information and signposting of clients to other organisations, either face to face or by phone.

7. To liaise with all referrers and highlight the service to health and social services and other interested parties in raising the profile of the scheme.

8. To provide written records of each visit, maintain confidentiality of the client and update customer database with all relevant information.

9. To ensure that the all client feedback is proactively gathered, evaluated and influences future service delivery.

10. To undertake relevant training to maintain skills and knowledge necessary for this post.

11. To comply with all Age UK Berkshire policies and procedures and with all relevant legislation. To comply with all provisions of the Age UK Berkshire Information Governance policy and procedures.

12. To carry out any other duties, commensurate with the rate of pay and grading, as may from time to time be assigned by the Trustees of Age UK Berkshire.
