By becoming a Pathway Navigator, you will be part of a community of similar roles across Greater Manchester. Working closely with the relevant Clinical Nurse Specialist (CNS), you will have a key role in ensuring that patients have a cancer diagnosis as early as possible, supporting those patients in the early stages of the pathway by being their first point of contact up until diagnosis. You will work with patients, their carers and health care professionals to assist in co
- ordinating the appointments and information required by clinicians in the pathway to deliver the faster diagnosis standard.

The post holder will support the implementation of personalised Care for people affected by working with the clinical nurse specialists when implementing personalised Care Interventions including:

- assisting with and/or delivering holistic needs assessments and producing associated care and support plans
- Personalised Stratified Follow-up, e., risk-stratified remote monitoring and surveillance
- Provide a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist need
- Work collaboratively with the Cancer CNS within the Tumour specific cancer team to provide a seamless service, which is responsive to individual patient’s
- Ensuring timeliness of investigations with appropriate follow up appointment based on clinical need as assessed by the Cancer
- Ensure the patient understands their care pathway and is aware of appointments and any changes.
- Support the clinical nurse specialist with the administration of holistic needs assessments and care
- Provide support to people affected by cancer, often dealing with highly emotional and sensitive issues and have an ability to manage individual situations effectively

- Ensure the patient understands their care pathway and is aware of appointments and any changes.
- Complete holistic needs assessments and care planning under the supervision of a registered practitioner and complete any necessary admin associated with this.
- To monitor time intervals between tests/treatment, pre-empting and identifying bottlenecks or delays in the patients’ pathways, escalating issues where necessary and highlighting delays.
- Monitor investigations and tests to ensure they have been completed and results/reports are available prior to appointments and consultations and MDT meetings with the clinical.

If successful this vacancy will require a Standard DBS check at a cost of £21.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
