**We are currently recruiting**:
Calderdale strives to work in an integrated way, particularly with our local health services. Within Calderdale, the Hospital Discharge Team, as part of the Early Help and Prevention service, support in the discharge planning for the people admitted to Calderdale Royal Hospital and Huddersfield Royal Infirmary. The team is made up of Local Authority social workers, service coordinators and provision planners but also works very closely with health colleagues within the NHS. The role of the provision planners is varied, a provision planner will work alongside service coordinators, social workers and discharge coordinators in the hospital to facilitate safe and timely discharges. This can include sourcing packages of care or care home placements depending on discharge destination. Provision planners also support palliative care work and brokerage function if someone requires amendments to their support plan. Developing relationships with care homes and care providers is an essential part of the role.

The Provision Planner position is a valued and vital role that provides a valuable element to discharge planning and supports to reduce pressure on a variety of teams as an admin function.
Provision Planners are typically in high demand, making sure the conditions are created for a safe and timely discharge from hospital, as well as reducing pressure on other areas of the service.

Working with individual people means the role can very varied, where no two days are ever the same. As a provision planner you will have a pivotal role as part of the hospital discharge team in supporting assessors to follow up with the discharge process.

**What we are looking for**:
Ability to relate to people Your role as a Provision Planner is to have direct contact with people, usually on a face-to-face basis. To join our existing teams, you will have an expert ability to quickly develop a trusting relationship with people, utilising a kind, empathetic and friendly approach. You must be able to not only listen deeply to what people are telling you but also be skilled at adapting your own communication to ensure you get the best out of the interactions you have with the people who call upon our support.

Resourcefulness and creativity You will recognise that formal services are not always solutions. You will need to be useful, resourceful, and creative when it comes to drawing on the strengths of our people, their wider networks, as well as your local knowledge of the community assets available to problem solve and meet their requirements. You will need to be confident to challenge the status quo or preconceived ideas to enable this happen.

Adult Social Care Knowledge The type of person who will do well in this role will have experience and knowledge of adult social care. You may have direct lived or living experience of adult social care yourself, or maybe involved or close to someone who has. You may be a care worker or in a similar role and looking for your next career move. Whoever you are and whatever your experience is, it will provide you with a sound platform to build upon to fulfil the role of a Provision Planner and we would love to hear from you.
