An opportunity has arisen for a Band 3 Administrator in a multi-disciplinary team, working with other administrators and health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
As an Administrator working under the general direction of the Senior Hub Coordinator undertake a range of administrative duties. Develop and maintain administrative systems, procedure and records to enable the team to delivery effective community services and to ensure that the office environment is safe, appropriately maintained and equipped. You will be required to participate in a rota to provide cover between 08:00 and 19:00 Monday to Friday, also weekend working on a rota basis. There is an aspiration that the hub operating hours will extend to cover 7 day working 08:00 to 20:00 at some point in the future. You will be required to be flexible and able to provide cover for other teams, this may involve working at different locations within the Locality.
Please note that there will be weekend working on a rota basis.
- Participate in a rota to ensure the hub has administrative cover between 08:00 and 19:00 on weekdays; this may change to 7 days 08:00 to 20:00.
- Carry out activities to support clinical triage of referrals for community care and track patients through care pathways across primary, secondary and acute settings.
- Schedule visits to patients on behalf of Clinicians and Assistants using electronic systems where available.
- To answer telephone calls as appropriate of the protocols of the service
- Input and retrieve data using IT systems, ensuring that all information taken is recorded quickly and accurately.
- To communicate effectively and consistently both with colleagues and callers to the service whilst retaining the ability to remain calm in difficult situations.
- To prioritise calls requiring immediate attention of a clinician or outside agency such as NCUHT and NWAS.
- Recognise and action needs of callers and GPs requiring other services e.g. Mental Health Crisis Intervention team, Social Services and Third Sector.
- Prepare reports and letters in relation to patient care.
- Respond appropriately to enquiries from other services and patients, referring to other members of the team when necessary.
- Ensure adequate clinical and office supply stocks available for teams.
