Band 4 Therapy assistant Practitioner

Discharge Team 37.5 hours

Due to significant new investment within Therapies, we are excited to be advertising posts for a new Discharge Assessment Response Team (DART). The main focus of this new team will be to support the Trust's priority of reducing length of stay, facilitating early discharge and improving patient flow, as well as timely responses to requests from senior management.

One of the main priorities for this new team will be to ensure patient centred care and patient satisfaction.

The post holder will need be motivated, dynamic and committed to this post, and have proven expertise within Therapy and supporting complex discharges. Promoting the service within the Trust will be vital to its success and requires proven advanced communication and interpersonal skills.

Every effort will be made to accommodate special interests and all staff will be involved in Professional/Peer group meetings with opportunities for continuing professional development.
