We are looking for a resettlement case worker to offer ongoing support to individuals upon discharge from the hospital. The worker will address the individuals' support needs and work collaboratively with the hospital and community services to meet each individual's support needs to improve mental health.

**PURPOSE OF THE POST**
- Day to day delivery of casework and provision for cover for the service across the Adult resettlement team.
- To work confidently with the Hospital pathway flow and discharge teams, and Case Manager in promoting quality customer care and good quality services (to include consultation meetings and events for your area).
- Utilise the Bridge to Homes intervention budget responsibly, keeping all records of expenditure and receipts and submitting expense returns/invoices on time to the Spring North Head of service.
- Undertaking of assessments for participants referred to the service, maintaining a high standard of case management and recording using the ZOHO system, (in line with service protocols).
- Ensuring performance is in line against contracted KPI’s for this project. (Key Performance Indicators).
- Ensure the service meets the requirements, and that all quality standards are maintained.
- Contribute to towards operational developments, including organisational objectives identified through the annual business planning process.
- To be an active & effective member of our project team and deliver monthly updates, reports and briefings as required.
- Develop and maintain relationships with a wide-ranging network of statutory and voluntary partners to ensure a coordinated response to participant needs.

**2 PRINCIPAL DUTIES**
- Develop & support innovative service methods & approaches to best meet the needs of project participants that we support.
- Ensure that all service practices conform with our safeguarding policies & procedures & Local Safeguarding Board’s Assurance teams.
- Work with project participants & the community to encourage & support their contribution to service developments and participation.
- Support the dissemination of information to project participant
- Work to deliver integrated services for this and any interlinked projects, as appropriate.
- Ensure the safety of staff, volunteers & project users in line with Health & Safety guidelines.

**3 ADDITIONAL RESPONSIBILITIES**
- Represent the organisation at meetings, conferences and exhibitions as required
- Provide a non-discriminatory service and treat all who access the Adult resettlement services fairly and equally.
- Be aware of the Health and Safety requirements in the workplace, and in particular, as it relates to own service area and/or department.
- Keep accurate records, provide written reports and quarterly funding returns as required by the funder or commissioner.
- Contribute to our operational and strategic development, including any organisation objectives identified through designated quality standards.
- Participate fully in personal supervision meetings and the annual appraisal process and undertake any training deemed necessary for the role.
- Review own delivery performance against agreed annual KPIs (key performance indicators), aiming to meet or exceed personal targets/performance levels.
- Undertake additional duties, training and/or hours of work as may be reasonably required which are deemed appropriate to the levels of responsibility within the role.
- Have a personal responsibility to ensure that person identifiable, confidential or sensitive information is processed in line with the General Data Protection Regulations and complies with the law, organisational polices and best practice when processing information.
- Work outside of normal office hours (i.e. evenings and weekends) as and when required to meet the needs of service users as appropriate.
- Undertake other appropriate tasks as may be required of the post holder from time to time and provide cover for tasks during periods of sickness or holiday.

**Job Types**: Part-time, Fixed term contract
Contract length: 12 months

**Salary**: £24,500.00-£26,500.00 per year

Expected hours: 30 per week
