**JOB DESCRIPTION**

**JOB PURPOSE SUMMARY**:
**RESPONSIBILITIES / ACCOUNTABILITIES**:

- To provide high quality coordination and support to families of children with neurodiverse traits ensuring they receive timely multi-agency coordinated care
- To act as point of contact for the families and involved agencies, who will assess, review and monitor coordinated support using appropriate assessments to ensure positive and sustained change for families
- Work closely with other agencies to include membership of multi-disciplinary teams, updating them on pathway progress and/ or when a change has occurred and link with adult services where parents are receiving adult service support.
- To support children and families to navigate the patient pathway thereby reducing any delays for the patient and working toward the patient being seen at the appropriate time.
- To ensure the delivery of high standards of patient care at all times, in line with Trust policies and guidelines.
- To provide teaching to the wider member of East and North Health and Care Partnership(HCP)
- To work as an effective member of the multi-disciplinary team.
- To work within the boundaries of the role and refer all other matters outside of their scope of practice to the appropriate professional.
- To assist in the reduction of NBI rates.
- To work in partnership with multiple agencies to ensure patient navigation of pathways is effective and efficient
- You will support and educate patients and families in navigating health and social care systems.
- To answer the telephone in a professional manner and refer callers onto the appropriate member of staff as necessary.
- To ensure that the dignity and respect of the patient is maintained at all times, supporting the Trust’s values and behaviours.
- To maintain accurate documentation and ensure the patient electronic clinical record is updated
- To ensure other agencies are updated on pathway progress on a regular basis and/or when a change has occurred
- Effectively communicate with patients/families and carers.
- To provide general, non-clinical advice, information and guidance to families, children and education settings within the boundaries of the role.

**CANDIDATE REQUIREMENTS**:
You will have a genuine respect for neurodivergent people, and a profound commitment to improving the experience of neurodivergent CYP and their families
- You are required to hold a full, valid, UK driving licence and have access to a car to use with business insurance (unless the member of staff has a disability as defined by the Equality Act 2010).

**PERSON SPECIFICATION**

**Essential criteria**:

- NVQ level 4 or above in child care, social care or health care.
- Experience of working with parents/carers
- Understanding of the roles of other professionals
- Able to prioritise workload
- Experience of working within community services, voluntary or statutory organisations
- Knowledge of financial benefits available & services provided by statutory and voluntary agencies to families of neurodiverse children and young people
- Knowledge of child development
- Knowledge and/or experience of safeguarding
- Good IT skills
- Able to reflect on own practice
- Good organisational skills Personal Qualities
- Good interpersonal/communication skills including listening
- Negotiation and problem solving skills
- Able to work independently and as part of a team
- Enthusiasm and commitment
- Approachable and sensitive

**Desirable Criteria**
- Any additional qualifications relating to children with neurodiverse needs
- Experience of working with families in their home
- Experience working across different teams

**Job Types**: Part-time, Fixed term contract
Contract length: 18 months
Part-time hours: 20-24 per week

**Salary**: £25,000.00-£30,000.00 per year

Schedule:

- Monday to Friday
- No weekends

Ability to commute/relocate:

- Hitchin, Hertfordshire: reliably commute or plan to relocate before starting work (required)

**Education**:

- Certificate of Higher Education (preferred)

**Experience**:

- Training: 1 year (preferred)
- Health and Social Care: 1 year (preferred)

Licence/Certification:

- NMC (preferred)
- Driving Licence (preferred)
