In particular we serve the area known as ‘Little Portugal’ (the most concentrated area of Portuguese speaking people outside Portugal, in Europe), a growing Spanish Latin American population, Caribbean, West and North African populations. At the same time there is an evolution taking place in our demographic, bordered by Clapham, Kennington and Battersea - there is an increasing number of young professionals moving in. Themes of diversity and identity drive our strengths as a PCN and we are pioneering new models of primary care within Stockwell that will intrinsically embed our community and how we deliver health services. Made possible by building on Stockwell’s strong presence of active and well engaged community organisations supporting the population in a variety of wellbeing needs including social support, housing, women and children, faith and education..

**JOB TITLE: StockwellBeing PCN Health Inequality Link Worker**

**HOURS: 22.5 - 30 hours per week**

**SALARY: £27,000 pro-rata onwards depending on experience**

**LOCATION: Grantham Practice, Grantham Road, SW9 9DL**

This is an exciting new role for someone who is passionate about working with communities to address some of the health inequalities faced an inner-city London. Unfortunately we know that peoples risk of suffering from conditions such as mental health, heart disease and chronic pain is greater if you are struggling with social and financial issues, or come from different languages and culture.

You will be working with GP surgeries in Stockwell, Lambeth helping them to engage with and support local people finding new ways to help them look after their health.

This means you partly work on site in GP surgeries with clinicians, getting support and training in looking at health data, contacting and developing relationships with patients. But you also get out into the community, build links with our local partners on the ground, helping to deliver projects in community venues around health and wellbeing, helping us as clinicians understand better the needs of our population.

As the health inequality link worker you will be working under the direct supervision of the Clinical Stockwell Primary Care Network Health Inequality Lead. You will be part of a team with social prescribers and finance support link workers. You will also work closely with our exciting new community movement - Thriving Stockwell and get to know and work with their community connectors.

You do not need any previous experience in a clinical setting for this role. However ideally you would be someone who has worked in the voluntary and community sector, working on the ground supporting vulnerable and/or under-served individuals and groups. You would have excellent people skills and enjoy networking. You would also be able to work from various sites within the Stockwell neighbourhood and transport between them.

**Main Duties and Responsibilities**
- Support the PCN to identify unmet health needs within the community and gaps in healthcare provision; The major area of focus will be Cardiovascular disease (i.e. Diabetes, Blood Pressure, Stroke)
- Identify and Capture feedback by engaging with community partners / residents on the key drivers of health inequalities they encounter
- Work with the HI Lead and TS to develop strategies to tackle drivers of inequalities, including outreach (in community venues) / in-reach (at GP surgeries) activities;
- Help deliver and develop one of our core health inequality projects - remote Blood pressure monitoring in community venues which supports those that are at highest risk of inequality in controlling their health at home.
- Become comfortable handling GP data to identify cohorts of patients that need our support (training provided as needed).
- Contacting patients and introducing them to projects we design
- Helping to deliver projects by developing some basic skills such as blood pressure taking and/or delivering basic healthy lifestyle advice (training will be provided as needed)
- Help coordinate group / peer support activities that run in community venues
- Develop and maintain close links with the GP practices of SWB to help liaise between them and community partners
- Work with the HI Lead to record and present data to the PCN in order to communicate progress and share learning of HI activities (a quarterly newsletter)
- Understand and be able to communicate the processes and procedures of GP surgeries (e.g how to register with a GP) in order to advise and help community partners navigate how to best support their clients;
