**6 MONTHS FIXED TERM/SECONDMENT/FULL TIME**

**An exciting opportunity has arisen within Perth & Kinross Health & Social Care Partnership for a Band 6 Registered Nurse or AHP equivalent within the Discharge Hub based in Perth Royal Infirmary.
**The Discharge Team lead and co-ordinate complex discharge planning as an integrated multi-disciplinary team allowing for safe, effective and timely discharges in order to maintain capacity & flow. **Person centred assessments are carried out which address health and social care needs to support and facilitate discharge planning across 7 days. You will work alongside multi-disciplinary and multi-agency teams across traditional, professional and service boundaries with the ability to adapt and manage change and promote joint working.

**You will show continuous professional development, be able to demonstrate leadership and excellent communication and interpersonal skills.**

**There will be an expectation that the post holder will actively participate in creatively developing the service.**

You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the NMC Register or AHP equivalent
