We are looking for passionate and driven people who want to join our Oxfordshire-based team to work within the local community. You will work as part of a team of passionate health coaches and advisors in the community outreach NHS Health Checks service across Oxfordshire.

You will deliver comprehensive NHS Health Checks to members of the public aged 40-74 years to provide a detailed assessment of their current state of health and to refer them and signpost them to relevant services to improve their health and wellbeing post check.

You will deliver checks in community venues and workplaces.

This role will work to agreed targets and objectives as part of the team to ensure that the service delivers what it needs.

The role is ideally suited to people with strong communication skills and those who are really passionate about making a difference to people’s lives on a daily basis.

This role would be best suited for those who have previous experience in being a health care assistance within health and social care.

**This role is full time** & all training will be delivered.

Salary £22,000 - £25,000
