We have a vacancy in our well established and respected Wellfamily Service for a highly motivated self-starter with excellent communication skills. Family Action has been providing our Wellfamily Service in Swaffham for more than 20 years.

At Family Action we transform lives by providing practical, emotional and financial support to those who are experiencing poverty, disadvantage and social isolation. We have been building stronger families since 1869 and today we work with more than 60,000 families in over 150 community based services, as well as supporting thousands more through national programmes and grants.

We are looking for a Wellfamily Practitioner, to work with the Wellfamily Service Co-ordinator and the Norfolk and Suffolk Service Manager to continue to develop the service. This would be an ideal personal development opportunity for someone who is keen to pursue a career in child and family support work and/or the mental health field. We are looking for a motivated and passionate worker with previous experience of delivering high quality 1 to 1 services to meet the needs of local communities in a professional or voluntary role. You will strive for excellent standards, robustly ensuring a quality, outcomes driven and accessible service. By joining Family Action’s Wellfamily Service you will be providing a community-based approach to improve and support the emotional health and wellbeing of patients of all ages registered at GPs Practices in Swaffham, Downham Market and Litcham.

Your impact

You will work 1 to 1 with service users referred to Wellfamily, helping them to identify wider issues that impact their health and wellbeing. You will empower them by developing an Action Plan with them to help them make positive, long-lasting changes in their lives. You will be working with patients of all ages, adults, young people and children and their families registered at one of seven local GP practices.

Your skills

You will have a relevant qualification or equivalent experiences of supporting mental health and wellbeing with adults, young people and children, and have a good understanding of the issues that can have an impact on emotional development and mental health. Evidence of a commitment to continuing learning and professional development is key as well as an understanding of child development and attachment. You will also have good knowledge of other local services and agencies and their referral criteria and processes.

Importantly, you will have an ability to meet the needs of service users sensitively and responsibly and demonstrate excellent listening and communication skills

Your primary role would be to contact and gather information about all adult, children and families referred to the Wellfamily Service within 6weeks.To signpost or refer to other services as appropriate.To undertake holistic assessment of cases allocated to you.To maintain a small case load of adult, children and family cases and to offer a range of short term interventions

You will work within the Wellfamily team, alongside other Family Action services and teams in Swaffham, Downham and Litcham Primary Care Network, and in partnership with other local agencies and partners.

Pay: £24,004.00-£26,439.00 per year

Expected hours: 12 per week
