We are looking for a Band 4 Nursing Associate to join our established team

The Intellectual and Developmental Disabilities Service provides specialist NHS services to people, where mainstream services cannot meet their current needs. The Orion Unit is a 8 bedded inpatient Assessment and Treatment service for adults with an Intellectual Disability and complex mental health difficulties and / or challenging behaviours that cannot be met within the community. We offer specialist support tailored to the individual, drawing flexibly on a broad range of models and interventions - working with a highly skilled Multi-disciplinary Team. The Unit helps and cares for people in times of crisis, difficulty, and distress; effectively collaborating with professionals, the person, their family, and networks that support them - including community colleagues and services.

This is a progressive developing role role, where the Nurse Associate will work and support the nursing team and wider MDT, working in a person-centred way, ensuring plans of care are implemented and followed accordingly by the healthcare support team.

You will have the skills and knowledge needed to work closely with the team, patients, and external services, to identify opportunities and resources for recovery - whilst engaging in learning and reflective practice opportunities to maintain and develop these skills.

You will work as part of Nursing alongside the Multi-disciplinary Team, to provide and support excellent and person centred care - whilst having the opportunity to contribute and develop high quality assessment, formulation, implementation and evaluation of patients’ needs.

You will have the skills and knowledge needed to work closely with the team, patients and external services, to identify opportunities and resources for recovery - whilst engaging in learning and reflective practice opportunities to maintain and develop these skills.

As a Nurse Associate on the Orion Unit, you will work closely with the Nursing Team in ensuring that the care and recovery pathways of our patient are of excellent standard.

**These roles, responsibilities sand tasks will include**:
To be an Accountable Professional
- Effectively communicating with the Nursing team, in both planned and ad-hoc meetings, to handover any relevant information and organise patient care.
- Ensure that Physical health needs are maintained and when needed, the ability to assess and escalate as appropriate.
- Leading on the maintenance and review of Physical Health Care Plans.
- Contributing to and supporting the plans and requests of the Multi-disciplinary team e.g from Ward Round Actions
- Ensuring patient and ward monies and stored and recorded accurately.
- Representing HCA’s within MDT meetings.
- Supporting in the development, review, update and patient involvement of Care Planning and ensure that these are recorded as appropriate.

To Promote Health and Prevent Ill Health
- Assessing and escalating physical health observations - communicating and recording any information or feedback as appropriate.
- Supporting in the promotion and motivation of patient’s personal hygiene and wider ADLs.
- Supporting with medication, such as being the 2ndsignee for CDC.
- Supporting in the maintenance and collection of physical health data, such as venepuncture and sample collections.
- Using the above information, to contribute to and update the appropriate care plans.

Provide and Monitor Care
- Support in completing physical health observations and communicating and recording these as appropriate.
- Escalating any concerns to the appropriate professional, such as Medic, Nurse or AHP.
- To attend and contribute to review meetings, such as CTR and CPA.
