The post-holder will work alongside 2 specialist diabetes dietitians and a dietetic assistant to provide dietetic care to adult with diabetes in primarily in community settings. The dietitians work from Airedale Hospital and also within the diabetes centre, GP practices and community resource centres within the Airedale NHS Foundation Trust locality. The service operates Monday to Friday but the post holder may be required to work out of normal office hours at times to support service developments.

The dietetic diabetes caseload includes adults with Type 1, Type 2 diabetes, Gestational Diabetes and other rarer forms of Diabetes. The team work collaboratively with the Primary Care Networks and secondary care providers across Airedale, Wharfedale and Craven to deliver a seamless diabetes service across primary and secondary care.

The post holder will also provide a service to the Dietetic offender health service covering six clinics per month in this innovative service.
- To lead and develop the dietetic diabetes service.
- To be responsible for planning, coordinating, delivering and evaluating clinical case work within the dietetic diabetes team.
- To ensure evidence based, highly specialist knowledge and skills is provided to the patients referred into the dietetic diabetes
- To provide a key role in partnership working with patients, carers, the MDT, and wider statutory, private and voluntary sector services.
- To take responsibility for effective functioning of the team.
- To take a lead with quality improvements in dietetic practice on the specialty of diabetes
- To take responsibility for staff and student training and development.
- Provide support to operational and service managers deputising for them as required

In pursuing these duties the post holder will ensure compliance with the NHS Constitution and the Trust’s Right Care strategy.

Clinical
- Ensure the team provides a timely and efficient clinical service to patients with reference to service priorities and responding to the changing needs of the team and the service.
- Ensure that all patients receive a high standard of clinical care from the team and that all team members meet the standards expected from the British Dietetic Association (BDA).
- Provide clinical interventions to patients as required.
- Collaborate with the MDT to develop and improve patient pathways.
- Respond to unforeseen situations that take precedence over planned work.
- Provide expert advice, teaching and instruction for relatives, carers, and other health professionals to promote understanding of the aims of the dietetic diabetes team and to ensure a consistent approach to patient care.
- Provide highly specialist advice to therapy and dietetic colleagues working within other clinical areas across the Trust.
- Train and supervise junior dietitians, assistants and students. This will include the planning the diabetes block of the student training programme.
- Use highly developed communication skills to establish good working relationships with colleagues, service users and carers.
- Use verbal and non-verbal communication skills and sensitivity with patients, relatives and carers to overcome barriers to communication e.g. loss of hearing, altered perception, expressive and receptive dysphasia, pain, anxiety, tracheotomy, problems with cognition.
- Deal effectively with complex, sensitive or contentious communications

**Specific**:

- To act as an expert source of advice on the dietetic management of diabetes to patients, carers, the multi-disciplinary team and other health professionals.
- To act as an expert source of advice on the dietetic management of prisoners as part of the telemedicine service
- To lead, manage and support the senior diabetes dietitians and dietetic assistant
- To liaise with regional dietetic services to support the care of patients accessing these services living in the Airedale NHS Foundation Trust location
