Clinical Incident Investigator

The Patient Safety Team work across all services in the trust, which include community, mental health and urgent care services. This role provides opportunity to gain knowledge of many different services/teams, and different types of incidents (although due to nature of our service serious self harm or suspected suicide are majority of reviews).
The Patient Safety Team is a small team and is proud of our principles of working in a supportive, considerate and learning culture. We support and engage with those effected by incidents to minimise harm and identify the learning and improvement that will have a positive and lasting impact on the safety of the care that we deliver.
The Trust is in process of planning transition to the Patient Safety Incident Response Framework and as part of implementation we have a new permanent role of Clinical Incident Investigator.
We are looking for an enthusiastic, organised, motivated person, with excellent communication skills. This is a challenging role within a busy team, but is rewarding to work collaboratively with clinical staff and patient/family to ultimately identify areas of learning and improvement. The Patient Safety Team currently work flexibly between the office and home and this role will require you to travel to different sites across our geographic area. The requirement of the role will be the completion of investigations and learning responses within mental health services and experience of reviewing care in this setting.
