**Care Co-Ordinator**

As a Care Co-Ordinator, you will be responsible for supervising our staff teams, ensuring that the best care is delivered to our service users and acting as first point of contact for the staff team.
- **Main Responsibilities**_
- Mentor new staff and support existing staff, building relationships with them to ensure that they have a comprehensive knowledge of service user’s needs.
- Carry out and review risk assessments, person-centred outcome-based care plans and hospital passport for service users and ensure the staff are following these.
- Follow and be aware of CQC and LA guidelines on Safeguarding Processes.
- Act in accordance with, and ensure all staff follow and comply with Maximus Support Services Ltd policies and procedures.
- Introduce and be actively involved with the support provided by our staff team.
- Complete bi-monthly spot check visits and direct observations to review performance and practices.
- Gain feedback from external providers we may be working with to ensure continuity of care.
- Work collaboratively with management to develop and motivate your team of staff, working closely with them and providing support and ensuring up-to-date training is arranged when required.
- Support with rota management when required.
- Take the lead in team and core team meetings.
- You will be given the training and support to embark on this rewarding role.
- Undertake on call duties on alternative weeks.
- Previous experience supervising and managing small teams.
- Possess excellent communication skills.
- Experience in carrying out risk assessments and care planning.
- Experience of supporting individuals with Learning Disabilities and Autism
- Sound knowledge of person-centred care
- Health & Social Care Diploma Level 3
- The commitment and dedication to make a difference to the lives of the service users.
- Knowledge of staff rostering systems and IT skills
- Applicants must have the right to work in the UK.

We offer:

- Salary £24,000 - £27,400 per annum depending on experience.
- Monday to Friday with flexibility on start and finish time.
- Additional on-call allowance.

**Salary**: £24,000.00-£27,400.00 per year
