Fracture Prevention Service Assistant

An exciting opportunity has arisen within the Fracture Prevention Team, for an enthusiastic and motivated individual to work alongside the Fracture Prevention Practitioner.
- To assist the Fracture Prevention Practitioner in identifying fracture patients on the wards, from fracture clinic and from the trauma lists, working without direct supervision. The percentage of identified patients is monitored nationally and published as a measure of the performance of FLS.
- To assess designated patients, within a defined care pathway, using agreed proforma, and at times tailoring more complex pathways without direct supervision.
- To provide comprehensive administrative support to the service
- To ensure that departmental clinical areas are maintained in a clean and tidy condition, and to be responsible for ordering stock and non-stock supplies for the service.
- Ordering patient information leaflets to ensure stock levels of essential items remain available at all times.
- Arranging patient telephone follow up calls to meet the service targets, i.e. all patient to be called at 3mths, and then again at 12mths post fracture, without supervision. The timing of these appointments are required to meet Nationally published targets.
- To attend education programmes/events for Osteoporosis and Fracture Liaison.

The post-holder will provide comprehensive support to the Fracture Prevention Service. The role is both patient facing and incorporates administrative functions with mínimal and often no supervision.
To work in conjunction with the Fracture Prevention Practitioner (Manager) in data inputting to generate the FLS-DB nationally audited reports to monitor all aspects of the Fracture Liaison Service working without direct supervision.
To support the Fracture Prevention Practitioner (Manager) as Trust wide point of contact for operations aspects this will include liaison with staff, Dr’s Consultants, GP’s patients and their carers and external companies.
- Patients and their relatives/carers, to elicit, receive and provide routine information, requiring questioning and empathic skills
- Medical and other clinical staff, administrative and support staff, both within and external to the Trust, to receive and pass on routine information.
- The post holder will have regular telephone contact with medical staff, suppliers and patients.
- The post holder will contact colleagues in other hospitals and primary care departments by letter and telephone.
Communication should be carried out politely at all times and will require the use of diplomatic, negotiating and analytical skills and the ability to empathise. The post holder will provide and receive routine information orally, in writing, electronically or over the telephone. Information may be of a sensitive nature and confidentiality must be ensured at all times. Any telephone and face-to-face enquiries will be dealt with efficiently and with understanding, knowledge of Osteoporosis medications and action taken as appropriate.
- Communicating daily with patients, both face-to-face and over the telephone. Eliciting, receiving and providing routine information, making appointments and dealing with queries. Will be required to give general advice to patients about aspects of the service/designated care pathway
- Following department policies and standard operating procedures in own role, may be required to comment.
- Required to follow Trust Policy
- Exercising personal duty of care in relation to equipment/resources
- Ensuring that stock levels (inc stationary & patient education materials) are maintained, ordering supplies as necessary
- Ensuring that departmental clinical, administrative and reception areas are maintained in a clean and tidy condition.
- Communicating daily with immediate colleague, other hospital staff, and primary care teams in the pursuit of their duties
- May be required to demonstrate own duties to new starters or other staff members.
- Recording personally generated information, inc. data entry & text processing
- Updating patient records
- Maintaining clerical and record keeping systems, both paper based and electronic, in line with established departmental and Trust systems, liaising with other departments, including Medical Records, where necessary.
May assist with collection and entry of data for surveys and audits - The collection of data for the FLS-Database in Nationally monitored and reported to the CCG 6mthly.
