Your key responsibilities will include:

- Be responsible for the assessment, support planning and review of complex individual needs.
- Signpost service users to the appropriate services.
- Provide triage/ first response services and where possible, promote opportunities for reablement.
- See safeguarding through from start to finish, ensuring that risk has been removed/reduced.
