The MND health care assistant is responsible for assisting the specialist MND practitioner to coordinate the assessment, treatment and support for people living with MND and their families within Swindon. To work with individuals across the community, within acute settings, Hospitals, Hospice and nursing homes, The prime purpose is to support the MND practitioner in carrying out assigned tasks involving care coordination and ensuring continuity whilst achieving the goals in caring for the complex effects of motor neurone disease. This will involve visiting and providing support and interventions in the patients own home or dwelling. Whats it like it to work here?
