Acting as a key member of the multi-disciplinary team, the post holder will provide assessment, psychosocial, evidence based interventions using a collaborative, recovery approach and will be skilled at positive risk management. The post holder will contribute to the clinical leadership of the team, providing supervision for other team members, as agreed with the Team Manager, and participate in team and service development and evaluation.
The post holder will provide a high standard of care and treatment for patients and work with other members of the team and other agencies to best meet the patients’ health and social needs.

- To provide day to day supervision and support to junior staff as necessary.
- To undertake the duty worker role.
- To plan and allocate visits to patients taking into consideration the safety of staff and patients and also the skill level required to provide the intervention.
- Responsible for the management of staff sickness including assessing the immediate impact of this sickness on staffing levels, rescheduling visits, and providing return to work interviews on return to work.
- To provide management supervision to other Registered Professionals, PDRs and ensure training is arranged an undertaken or to delegate to appropriate staff.
- To provide managerial and clinical supervision to other professional colleagues.
- To undertake audits of care plans of registered staff and provide guidance and feedback to the individual.
- To ensure the supervision of any trainees attached to the team is undertaken, including acting as a ‘mentor’ as necessary. (This includes trainee Assistant Practitioners, students and Nursing Associates).
- To ensure induction and in-service training of staff is undertaken.
- To work to a flexible rota based on annualized hours and collaboration with colleagues to ensure all shifts are covered.
- To escalate any concerns as appropriate to your line manager and keep line manager informed of any events or issues.
- To contribute to and be responsible for implementing any service changes as guided by national guidance.
- To provide good lines of communication within the team and external to the team for patients, carers, staff and other agencies.
- To be responsible for responding and addressing areas of concern/PALs queries brought to your attention by staff, patients or carers and keep line manager informed of any such issues
- To assist in the process of staff recruitment as required.
- To be actively involved in promoting the service to other professionals and community colleagues.
- To deputise for the Older Persons’ Mental Health Team Manager as required.
- To deputise for the Older Persons’ Mental Health Team Manager at Trust wide/ Divisional meetings
Information Resources & Administrative Duties
- To ensure high standards of record/case note recording in line with the Policy for Integrated Records.
- To recognise the importance of working as a team and sharing of information about Service Users under the care of the team.
- To observe all legal and statutory requirements with regard to the patient and their relatives with particular regard to confidentiality.
- To develop good relationships and working arrangements with other professional staff and agencies, to ensure the smooth delivery of care for patients.
