Care Navigator

Job Summary:
We are seeking a Care Navigator to join our team. As a Care Navigator, you will be responsible for receiving, assisting and directing patients to access and navigate the appropriate service or healthcare professional in a courteous, efficient and effective way.

You will provide support and assistance to the practice team as a whole and project a positive, caring, courteous and friendly image to patients and other visitors to the Practice, either in person or through the telephone system.

Duties will include:

- Receiving patients, ensuring marked as arrived at the correct site
- Handling documentation in letter form from patients regarding advisories from Out-Patient Departments, prescription requests etc
- Processing appointment requests via telephone or through the AskMyGP online service
- Cancellation of clinics and reappointing
- Care navigate patients appropriately to alternative services
- Process precription requests
- Call handling - answering phone in a positive manner and care navigating patient effectively to the correct clinician or service

**Skills**:

- Excellent communication skills, both verbal and written
- Strong interpersonal skills with the ability to build rapport and establish trust with patients
- Empathy and compassion towards individuals facing health challenges
- Organizational skills to manage multiple tasks and prioritize effectively
- Attention to detail to ensure accuracy in documentation
- Proficiency in using office software (e.g., Microsoft Office Suite)
