**Paramedic - General Practice**

This is an exciting opportunity for a Paramedic, to augment an already thriving, friendly and supportive Primary Care Team. You will be working as part of a multidisciplinary team, alongside seven GP partners, one ANP Partner, two Paramedics one minor illness nurse and three Salaried Doctors who are all complemented by an excellent Nursing Team and an extremely supportive Administration team.

The Paramedic will be responsible for acting within their professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care under the supervision of a GP. The Paramedic will demonstrate safe, clinical decision-making and expert care for patients within Atlantic Medical Group, in consultation with the duty GP.

**Main duties of the job**

As a paramedic, you will need to manage a clinical caseload and deal with presenting patient’s needs in a primary care practice setting as well as home visits when required. You will be a first point on contact for the patients presenting with undifferentiated, undiagnosed problems, utilising history taking, physical examination, problem-solving and clinical decision-making skills to establish a working diagnosis and management plan working in partnership with patients.

Providing a clinical leadership where appropriate within the practice and support other members of the team to develop and maintain clinical skills appropriate to your role and expertise is essential.

Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support with an emphasis on prevention and self-care. Refer patients directly to other services or agencies using appropriate referral pathways when necessary.

Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes in accordance with relevant standards.Work in accordance with internal administrative systems relating to but not limited to the management of clinical data. Send and receive written information on behalf of the practice relating to the physical and social welfare of patients. Work closely with other clinical staff and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets and work to deliver the NHS contract pertinent to the practice including the terms of the Quality and Outcomes Framework and locally enhanced services.

This role will also be actively involved in clinical research.

**Overview of the organisation**

Atlantic Medical Group is made up of 3 surgeries based at the very west of Cornwall. We have seven Partners, six of these being GPs and one ANP partner. We have three sites, one being a purpose-built new building shared with two other practices that opened in February 2019, a second site being in St Just, a beautiful town, and a third being in the busy fishing village of Newlyn. We have a list size of 11,500 patients and are part of a PCN that combined has 68,000 patients.

We are a training practice for registrars, medical students, physician associate students & nursing students and as such are a supportive team. We are the first training hub in the Southwest. We run six weekly educational / clinical governance meetings.

As well as the above, Atlantic Medical Group thrive in Academic and Commercial Research and we are currently involved in 7 clinical research studies.

**Job Details**
Telephone triage patients with undifferentiated and undiagnosed problems and assess their healthcare needs.

Receive patients in clinic and Home Visits. Assessment, diagnosis, treatment, prescribing, referral as appropriate. Providing public health, health protection and promotion programmes that improve health and reduce inequalities.

Supporting our team with homes visits to patients who are appropriate to be seen by a Paramedic
- in their own homes or in a care home setting. Carry out health reviews, medication reviews, care planning.

**Qualifications**
BSc in a training programme approved by the College of Paramedics.

Health & Care Professions Council (HCPC) registration

Desirable - independent prescribing

**Knowledge**
Desirable criteria
Knowledge of EMIS would be an advantage
