The Complex Discharge Team have some exciting news

We are looking for a Band 5 to coordinate a new Discharge Unit. The unit will be opening in March and will specialise in providing holistic and compressive discharge assessments of patients with complex needs.

The aim of the unit is to optimise patients' independence following a period of acute ill health and hospitalisation and promote home as the preferred discharge destination.

The applicant must have an in-depth knowledge of complex discharge processes. We are looking for someone who has the skills to promote and encourage a culture of empowering patients to continue their ongoing care at home.

**Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.**

This post is designed to support the role and the functions of the Integrated Hospital Discharge Team. This is a section within the Complex Discharge Team

Work using own initiative within a specialised team whose role will be to coordinate the care and services for patients with complex needs leaving hospital. The team will promote effective utilisation of resources and optimal outcomes for each patient across the continuum of care.

The job role will require leadership and management for the supporting administrative staff, developing a flexible workforce to meet the ever changing needs of the service within a managed financial envelope.

The job role will deliver expertise with regards to access targets for the patient pathways, analysing demand and capacity to maximise use of resources and eliminate waste.
- Provide business support to the Integrated Hospital Discharge Team.
- Manage the coordination of resources to adequately cover all whiteboards - this requires knowledge and understanding of caseload pressures according to ward areas.
- Advise and provide support to Discharge Case Managers (DCM’s) to ensure they adhere to process and discharge standards according to Care Act and National Discharge Guidance
- Coordinate all Complex Discharge referrals - linking previous support packages to support MDTs in discharge planning.
- Manage the day to day recording and verification of delayed transfers of care project/service.
- Manage failed discharges and find appropriate solutions.
- Manage the day-to-day recording, verification and reporting of Delayed Transfers of Care
- Ensuring the delivery of seamless high-quality service to patients and clinical teams
- To provide managerial and administrative support to the Discharge assessment and integrated hospital discharge team managers
- to include sickness, appraisals and ensuring appropriate levels of staffing day/night.
- To take responsibility of investigating and answering complaints.
- To support the managers with staff training and development.
- To support the DAU to maintain patient flow from the acute hospital to their pathway destination, to escalate any barriers to the appropriate organisation.
