Family Link Worker

As a Family Link Worker (FLW) you will be the link between the PCN and families with children under 5 registered with the GP practices in Greenwell PCN. The outreach work of the FLW is essential in addressing the rising inequalities in health and social care and identifying the unmet milestones in children of pre-school age. The FLW will perform a variety of duties with the aim of improving the health and wellbeing of the children in families identified. The FWL will play a significant role in increasing the early years development of the children through a range of interventions and advocacy, visiting the families in their home and building strong relationships.

The bridging role between health, social care and the community will facilitate improved access and supports the provision of services that meet the needs of identified children in the PCN. This is an exciting opportunity to shape a job role relatively new to the UK.
