Salary: Grade 3 SCP 7-12 = £24,294 - £26,421 per annum

30 Hours = 81.08% = £19,697 - £21,422 per annum

To work as part of a community based integrated Reablement team/Initial Support Services to undertake specific clinical and administrative tasks. This includes delivering intervention programmes as prescribed by the team Therapists. The interventions are to be delivered within the service users own home and community environment. Duties include being flexible to respond to service needs. Staff to work within their competency boundaries at all times, which are reviewed periodically by the team Therapists.
