To work as part of the Sheffield Eating Disorders Service Multi Disciplinary Team based at St George’s Community Health Centre.
- To work closely with qualified staff to implement agreed safe and effective physical health monitoring service users and carers under the supervision of a named professional.
- To support service users to achieve their goals, by supporting them with clinically approved safe practices as per NHS England and MEED guideline recommendations.
- To report any observations/untoward occurrences to a qualified member of staff to enable evaluation of care/reassessment to be undertaken as the service develops, we would be working with clients in their own homes to provide intensive support and care with their eating disorders.

The post holder will work within a developing Eating Disorders Service. They will provide physical health monitoring, practical, social and emotional support to clients aged 16 and up within the community, outpatient clinics and to those attending the Day Service. The support worker will work as part of a multi-disciplinary team including Consultant, Specialist Nursing staff, Psychology, Cognitive Behavioural Therapy, Dietetics and Occupational Therapy.

- Undertake a range of practical tasks and duties in support of the Team and individual needs.
- To contribute to the physical health monitoring of our patients.(Phlebotomy and Physical health observations)
- To feel confident to be able to report untoward incidents and areas of concern as soon as they arise
- To document any relevant information in an electronic record following relevant policy and legislation
- Contribute to individual reviews and team meetings
- Aspire to instill hope and optimism

1. Duties

Each individual has different needs however the list below outlines just some of the practical duties that the post holder will undertake, support and enable people to/with:

- Identify/contribute to their own goals to improve their quality of life
- Contribute to their own care/support/recovery plans
- Support individuals to express their needs and wishes
- Assist individuals withactivities of daily living encouraging active

participation and independence
- Promote their and monitor their physical health as required and guided by senior staff.
- Actively engage in health promotion
- To support individuals with external activities/appointments under indirect supervision
- To structure time positively
- Initiate, maintain and strengthen familial and social relationships
- Increase self confidence
- Embrace new opportunities

1. Trainingand Supervision
- Undertake mandatory and appropriate training relevant to your role for example:
General Health and Safety

Basic/advanced Life Support

RESPECT Training
- Identify own training and developmental needs
- Identify areas of training need that are relevant to the team - and or required as part of this role - Phlebotomy
- Assist in the delivery of training in areas of personal experience and interest
- Participate in annual performance development reviews
- Participate in regular individual and group supervision
- Use supervision to understand, reflect, inform and improve your work practice
- Participate in the development of the service

- Work flexibly to meet service needs

1. Administrative responsibilities
- Keep clinical records up to date using Trust systems in accordance with legislation as delegated by the nurse in charge
- Support people to contribute to their own clinical notes
